Resource Specialist Duties/Responsibilities:
- Determines, either independently and/or in conjunction with clinicians, the resources that would optimally meet patients' and families' needs and make independent decisions about what services to recommend to a patient/family.
- Maintains a caseload with multiple patient contacts per day, including phone calls, paperwork, and face-to-face contacts.
- Researches best practices and programs to meet the needs of patients and functions as the primary health care contact for psychosocially complex patients when needed.
- Provide timely and appropriate documentation in patients' medical records using EPIC electronic medical records.
- Advocate on patients' behalf in written and verbal communication.
- Assists patients and families with such tasks as accessing entitlement programs.
- Provides appropriate medical information as requested by extended care facilities, Department of Transitional Assistance, etc.
- Conducts staff and community-based training on resources.
- Provides updates to staff on procedural requirements and changes required by state and local agencies.
- Documents and advises supervisor and clinical staff of obstacles to obtaining services that require social action.
- Develops liaisons with community and governmental agencies to facilitate the negotiation of complex systems.
- Completes Mass Health applications for the needed patient population.
- Coordinates community initiatives.
- Assists with completing State and Federal FMLA paperwork for Cancer Center patients.
Administrative Duties/Responsibilities:
- Creates and maintains a professional work environment that requires teamwork, professional demeanor and appearance.
- Works independently with minimal supervision and exercises appropriate judgement in setting priorities.
- Manages verbal and written communications; including telephone calls, communications/reports.
- Maintains and protects confidential issues pertaining to patients.
- Produces information, reports and presentations by transcribing, formatting, analyzing, inputting, editing, retrieving, copying, and transmitting text, data, and graphics.

Education/Experience Required:

- Word processing including Microsoft Office (Word, Excel, Publisher and PowerPoint), typing 50 WPM; spreadsheet/database creation and management.
- Strong customer service orientation and ability to project a positive, professional image.
- Strong interpersonal skills and excellent telephone communications skills.
- Strong organizational and filing knowledge.
- Ability to maintain confidentiality and function with appropriate independence, exhibiting good judgment.
- Demonstrates initiative to follow through on assignments with minimal direction; initiating sustained improvements in processes.

- Experience working in a healthcare setting preferred
- Associates degree or higher preferred, with written and verbal English skills at college level.
- License preferred: Licensed Social Worker or enrolled in program obtaining MSW/LCSW
- 1-3 years related work experience required.
- 2 years of related work experience without a related Bachelor's degree
- High School diploma or GED required. Bachelor's degree preferred in Social Work or Human Services

This job requires frequent bending, carrying, pulling, pushing, reaching, standing, stooping, walking and lifting over 10 lbs. There will be occasional need for climbing stairs, lifting in excess of 50 – 100 lbs and sitting. Constant use of hearing, sight and speech. Frequent use of smell and touch. Frequent keyboard and telephone use. Constant concentration and problem solving.
Pay Range:
$26.50 - $35.67The pay range listed for this position is the base hourly wage range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law. Compensation may exceed the base hourly rate depending on shift differentials, call pay, premium pay, overtime pay, and other additional pay practices, as applicable to the position and in accordance with the law.
